Search for Change is a not-for-profit organization that provides residential and vocational services to adults in
recovery from mental illness. The mission of the agency is based on the principles of psychiatric rehabilitation
and the goal is to assist individuals to identify and obtain valued roles within the community.
Payment Terms: SSI, SSD, Medicaid, and private pay
Eligibility: Individuals must carry a primary diagnosis of mental illness as defined by the DSM-IV.
A secondary diagnosis of substance abuse/chemical addiction will also be accepted. All referrals
are reviewed on a case-by-case basis.
Special Programs:
SFC offers 24-hour Community Residential, Apartment Treatment and Supported Housing Programs in Westchester
and Putnam Counties. Residential Services: SFC operates a number of OMH licensed residential programs located
throughout Westchester and Putnam Counties. These are transactional housing programs with the goal being to aid
individuals in making informed choices in living, working and social environments and teach the skills individuals
need to be successful and satisfied in those chosen environments.
Respite & Hospital Diversion Services: SFC provides a temporary stay (up to 14 days) bed which includes 24-hour
supervision and support for individuals who are experiencing acute symptoms and who might be at risk of inpatient
hospitalization. Also offers step-down services for individuals who are ready to leave the inpatient setting, but
require additional support and supervision to aid them with their transition back into the community.
Private Case Management: Services are offered to individuals not enrolled in any of SFC’s other
programs and living on their own, and include ongoing, in-home support, supervision, advocacy, linkage with treatment
and rehabilitation providers as jointly determined by participants, families and staff.
Vocational Services: SFC offers vocational rehabilitation services to assist people with psychiatric disabilities
to choose, obtain and maintain employment, vocational assistance to prepare individuals for a return to the work force,
and vocational training including computer training and internship program.
