The following is information sent in by providers in our area. It is not an endorsement of any particular provider or type of service. Contact providers directly with questions about services and costs. This page reflects information provided in September, 2004 and February 2006. There is a brief table containing basic details, followed by individual information about each provider that was too long to list in the table. Click on the name of the provider to be taken to their information. The providers are alphabetized by community.
Types of mental health providers:
Psychiatrist: a medical doctor specializing in mental health. They can prescribe drugs and monitor your meds. If you are looking for medication management, you need a psychiatrist.
AODA: Trained in dealing with alcohol and other drug addictions.
PhD and PsyD: Doctoral degrees in Psychology. This is the most advanced degree available in Psychology, and entitles them to be addressed as "Doctor." They have not been to medical school and so they cannot prescribe medications for you.
MSW: Masters degree in social work.
MS/MA: Masters degree in another field, such as nursing or psychology. Ask the provider what their Masters is in.
LMFT: Licensed Marital and Family Therapist.

Types of Services Provided: Must be a resident of Crawford County to be seen here. Outpatient mental health clinic that provides psychiatric evaluations, medication management, psychological evaluations, counseling services, emergency intakes and services, ongoing follow up and treatment.
Statement About Costs and Insurances Accepted: We accept any insurance that an individual might have. Patients are advised to call their insurance company prior to an office visit to make sure we are in their network. If an individual does not have any insurance or the insurance does not cover here, we do have a sliding monthly payment schedule that is looked at. This payment schedule helps us determine if the individual would have a monthly fee per the family's gross monthly income and the number of people in the family. Our regular fees are as follows:
Psychiatric evaluations: $240/hr, psychological evaluations $240/hr, medication management $60, counseling services $117-$123/hr

Statement About Costs and Insurances Accepted: Accepted by many insurances. We encourage our patients to call their insurance company to make sure Gunderson Lutheran is a provider. You cannot negotiate a reduction with your counselor.

"Our philosophy of treatment is holistically oriented toward the health of the whole person, the physical, mental and spiritual unity of the whole person." Psychological testing and interpretation, professional education and training seminars, and consultation services are available. A wide range of issues bring people to counseling.

Types of Services Provided: Community Support Program, Community Options Program for Iowa County, Outpatient mental health services, outpatient substance abuse services, developmental disabilities services, emergency services.
Statement About Costs and Insurances Accepted: "Fees for most services are on a sliding scale, based on your ability to pay. Service will not be denied if you are unable to pay. In many cases, health insurance, Medical Assistance, or Medicare will cover some or all of your bill. Consultation on insurance arrangements is available, and we bill insurance companies directly.

Types of Services Provided: We treat ages 4-99 for problems with depression, anxiety, anger, eating disorders, chronic pain, PTSD, and relationship problems. Our psychologist does neuropsychological evaluations, memory diagnostics, court evaluations and adult ADHD evaluations. Modalities used include cognitive behavioral therapy, EMDR, assertiveness training, relaxation and visualizations as well as hypnosis and talk therapy.
Statement About Costs and Insurances Accepted: Accept Alliance, Blue Cross/Blue Shield, fee-for-service plans and HMO point of service plans. Self pay clients are given a 10% fee reduction.
