Memorial donations can be a meaningful way to remember someone who has passed. Honorary donations can be a special way to recognize a loved one or commemorate a special occasion.
Sometimes individuals prefer that donations in memory of their loved one be made to a special page, created specifically with their loved one in mind. With a memorial page, you can login anytime to view gifts and messages made in your loved one’s name in real-time.
Even if you don’t make a memorial page, we can still notify you of donations made in honor or in memory of your loved one. Fill out the form below to receive notification of gifts made in your loved one’s name. Once submitted, an email confirmation will be sent to the email address provided on the form with additional details.

A memorial donation is a donation that is made in the memory of someone who has passed away.
An honorary donation is a donation that is made in honor of an individual, often celebrating a special event like a birthday, anniversary, or wedding.
Any donation to NAMI can be a memorial or honorary donation. To explore some other ways you can make your gift, please visit the ‘Ways of Giving’ page of our website.
On our donation form, “honoree” refers to the person being honored or memorialized.
On our donation form, “recipient” refers to the person you’d like to notify of your thoughtful gift.
Only the ‘Honoree’s First Name’ and ‘Honoree’s Last Name’ fields are required on our donation form.
Unless the person you’d like to notify has contacted us to share their information, it is likely that we will not be able to send a notification if you do not provide their contact info with your gift. As such, we recommend having the contact information (email address and/or mailing address) for the person you’d like to be notified ready as you make your donation.
It typically takes between 7-10 business days for NAMI to prepare and send your notification letter.
Notification emails are sent automatically to the email address provided in the “recipient’s email” field, once your gift has finished processing.
In notifications of memorial or honorary donations, your name and mailing address are included in the notification. If you choose to provide an additional message with your gift, that information will be included as well.
